Twenty one consecutive cases of posterior circulation aneurysms were operated in the department of Neruosurgery, CNUH from Jan. 1983 to Dec. 1992. They made up3.5% of surgically treated 597 cases during this period. Of the 21 patients, aneurysms arose at basilar artery bifurcation in 11 cases, at posterior inferior cerebellar artery in 5 cases, at the junction of superior cerebellar artery and basilar artery in 4 cases, and at vertebrobasilar junction in one case. Eleven patients had other intracranial vascular lesions, such as multiple aneurysms in 9 cases, other aneurysm and dural AVM in one case, and cerebellar AVM in one case. Seven cases had large aneurysms, sized from 12mm to 24mm, and remainder had small aneurysms. The surgical result was good in 16 cases(76%), and the mortality rate was 9.05%.
